Advanced Directive Form
An advance directive is a legal document that outlines a person's healthcare preferences in the event that they become unable to make medical decisions for themselves due to illness, injury, or incapacity. These forms are available here for those who do not have an Advanced Directive in place. If an advanced directive is already in place, you do not need to fill out these forms and we ask that you kindly bring a copy of your already in place advanced directive with you to your next office visit.
Purpose:
To ensure that a person's healthcare wishes are respected when they are unable to communicate their own decisions.
To provide guidance to healthcare providers, family members, and legal agents regarding medical treatments, interventions, and end-of-life care.
Importance:
Advance directives can provide peace of mind to individuals by knowing that their healthcare wishes will be followed.
They can help avoid conflicts and legal disputes among family members and healthcare providers.
They can ensure that a person receives the medical treatment that aligns with their values and beliefs.
It's essential to note that advance directives vary by state and may require specific legal forms and witnesses. It's recommended to consult with a healthcare professional or attorney to create an appropriate and legally binding document.
